About this tool

How it works

The Pharmacy First calculator estimates the consultation income a community pharmacy in England earns from the Pharmacy First service, given how many consultations it completes a month, and shows how that changes as the number rises. It is a way of seeing what promotion — a service page, a Google Business Profile listing, a counter conversation — is worth in money.

How the calculation works

Consultations per month × the consultation fee gives the monthly figure. Pharmacy First also carries a monthly fixed payment that depends on meeting a minimum number of clinical pathway consultations, and the threshold has changed more than once since launch; the calculator uses the fee and threshold values shown on the page, which should be checked against the current NHS England service specification and Drug Tariff before you rely on them.

Assumptions to check

  • Fees and thresholds are set nationally and change; treat the figures as an estimate and confirm the current values.
  • Only clinical pathway consultations count towards the threshold; minor illness and urgent supply referrals are paid separately.
  • Distance-selling pharmacies cannot provide the earache pathway and have other restrictions.

How to read the result

If you are below the threshold, the fixed payment is the first thing to secure and a few more consultations a month usually does it. If you are above it, each additional consultation is marginal income with the pharmacist’s time as the main cost.
